South Milwaukee High School Sports Registration

Please read all information below regarding sports participation requirements
before starting the online registration.

Sports Registration must be completed by a parent/legal guardian on file with the school District.
The online account may not be set up by a student using a student email (including 18-year olds).

Families with more than one high school student, should add all students into one family account.
Do not set up individual accounts for each student.

Upon login, go to the "Registration History" tab and click on "Register" to get started. 

If you do not currently have an account set up, you may set it up at any time, but will not be able to register
for a specific season until each season registration opens on the dates listed below.

2023-2024 SPORTS SEASON START DATES

All athletes must be registered prior to participating in any sport, including those with tryouts.
Athletes must register for each season, if participating in more than one season.

REGISTRATION REQUIREMENTS

A parent or legal guardian must complete the online registration, each school year for each season (Fall, Winter, Spring), for all students wishing to participate in athletics, including Cheer & Poms Dance teams. Registration includes showing proof of a physical examination clearing the student to participate in sports, proof of health insurance, acknowledging the requirements regarding concussions and eligibility to participate, providing emergency contact information, and signing off on a transportation waiver. Have all of that information on hand before you start the online registration.

WIAA Physical Examination Requirements

  1. If the student is taking a sport for the first time, or their previous physical was done before April 1st of the previous school year, then proof of a sports physical must be provided to the District, and must be signed by a doctor, physician assistant or nurse practitioner, and a parent or legal guardian.   2. Students must provide health insurance information. If no family health insurance is available, supplemental insurance may be purchased on your own, or from the school District through First Agency which must be applied for online at https://www.1stagency.com/. You must be approved for insurance prior to completing the online registration. The online registration process will require you to upload a scanned copy or picture of your insurance card or other proof of insurance.
  3. If the student is in the alternate year of a sport, and their physical date was April 1 or later for the previous school year, then a parent or legal guardian only need to complete the online registration. You will not need to provide another copy of the physical form.

SPORTS FEES

The South Milwaukee High School Athletic fee is $75 per student per WIAA sport with individual and family maximums. Fee waivers are not available for athletics. Fees are not paid at time of registration. Once final teams are set up the fees will appear in the student's Infinite Campus (IC) account, and can either be paid online through the IC account, or by cash or check only in the Athletic Office (checks payable to South Milwaukee High School).

CHEER & POMS: Please note that students signing up for Cheer or Poms Dance teams, must pay the $75 fee for each season (Fall & Winter), if they participate in both.
